Question list
A short question list for the conversation
Imagine the visit is about to start and you have a lot on your mind. Choose the questions that fit. Leave space to write down the answers in your own words.
Begin with what you want help with. Then work through the medicine, the people, the bill and the follow-up. It is fine to ask for a plain answer to an unfamiliar word.

Start with the problem, then the options
Fact
The Federation of State Medical Boards' May 2026 guidance says choosing a treatment from a menu is not enough on its own for a prescription.
The group brings together the boards that oversee doctors. Its report gives boards advice; it is not a national prescribing law.
- What do you think explains the problem I want help with?
- What options can we compare, including options without this medicine?
- What would make you decide against prescribing it?

Get the exact medicine and pharmacy
The US medicine-review agency is the Food and Drug Administration, abbreviated FDA.
As checked September 26, 2026, a medicine made to order, also called compounded, has no FDA approval. Before sale, the FDA does not assess its safety, how well it works or how well it was made.
- What is the full medicine name and form?
- Is this ready-made or custom-prepared?
- If custom-prepared, what need cannot be met by a ready-made product?
- Which pharmacy makes or fills it, and how do I check its permit?

Confirm the people and the whole price
A video conversation with a doctor or nurse can be telehealth: receiving care from a distance.
Some people with a care licence from a different state may register with the relevant Arizona board to give remote care to patients here, as of October 3, 2026. This is called telehealth registration. They must meet Arizona registration and care rules.
Some online weight-care memberships leave the medicine as a separate charge, based on pages checked September 26, 2026.
- What is your job, and what licence or registration gives you permission to provide care when I am in Arizona?
- Which Arizona board can confirm that licence or registration?
- What is the full cost, including later visits, tests and medicine?
- What changes after an offer ends, and how do I cancel?

End with a follow-up plan
The medical boards' May 2026 guidance calls for a way to raise questions after the health check.
- How will we tell whether this is helping the problem we discussed?
- Which unwanted effects matter for this medicine and my health history?
- Who answers a medicine, label or delivery question between visits?
- What happens next if I decide to wait?
